- First of all please visit ceouttarpradesh.nic.in
- Go to the Tab named as “For Voters”. Just place the cursor on For Voters Tab.
- Drop down menu will be comes under For Voters Tab. Click on Voter Forms.
- Just you can download the Form 6 Application Form.
- Fill the application as per guidelines and submit it to your nearest Tahsildar Office.
- Another Procedure apply online at nvsp.in website. Same as above procedure.
